We know that choosing a care provider for your family is a big decision, so please feel free to contact us with any questions or concerns as you prepare to make that decision. Here at Santa Fe Family & Functional Medicine, we strongly believe in preventative medicine, health maintenance and addressing the core imbalances that may contribute to various medical conditions. At your first appointment, your care provider will complete a comprehensive examination including a detailed review of your medical history. Following your exam, you will discuss with your provider their findings and develop an individualized treatment program which may include recommendations for diet, exercise and other lifestyle changes.
Please be prepared for your initial appointment by printing and completing the new patient forms. If you have medical insurance, please be sure to provide all requested information to assist us in the benefit verification process.
Please refer to our billing FAQ page for a comprehensive explanation of our billing policies. We highly recommend that you take the time to review this page before your first appointment. Payment is expected at the time of service. If you are covered by insurance, we will collect your copayment at the time of service, if applicable. As a courtesy, we will file claims on your behalf with your medical insurance company. If you prefer to file claims yourself, we can provide you with the proper documentation at your request. If you are not covered by insurance, we do offer a 10% discount to our patients who pay at the time of service.
Our office is fragrance-free and we do ask that you refrain from wearing any perfumes, after-shaves, lotions or other body products with a fragrance when you come in for your appointments. We also request that you give at least 24 hours notice for any appointment cancellations in order to avoid any applicable no show fees.

What Our Patients Say
When I first saw Dr. Gollub, half of my hair had fallen out as had all of my eyelashes. I had a horrible rash that looked and felt like a cross between measles and a sunburn. On several occasions during the previous year I had woken up with my eyes swollen closed. I had to use ice cubes to get them to open. I had been to see an allergist and two dermatologists.
The first dermatologist gave up after several sessions, telling me that “you might just need to learn to live with it.” The second dermatologist I saw was very sympathetic and convinced that he could find the cause of my problems; however, that was not the case. My husband finally recommended that I stop seeing specialists and consider seeing someone who could look at the big picture. He had been a patient of Dr. Gollub’s for many years and felt that Dr. Gollub might be able to provide some answers.
On my first visit to Dr. Gollub he listened very carefully to everything that I had been through and performed a thorough examination. He told me that he thought the root of my problems might be in my gut and ordered some tests to determine if that was the case. He also recommended that I stop eating gluten temporarily. Dr. Gollub prescribed a regimen of yogurt and other probiotics and recommended some dietary modifications. He did not prescribe any medications.
Within two weeks my skin had almost entirely cleared up and my hair stop falling out. Within a couple of months I was entirely back to normal. Dr. Gollub is a puzzle solver which is exactly what I needed. I have recommended him to all of my family and friends. I go to him for everything because I very much appreciate his big picture approach and his kind and compassionate treatment. Dr. Gollub is my doctor for as long as he continues to practice.
